Participate in a survey to help us learn more about student stress and wellness

We are conducting a survey for research purposes. This survey is being conducted to learn more about contributors to stress and wellness for students enrolled in Ohio State’s PharmD program.

Your participation in the survey is strictly voluntary and there is no penalty for non-participation. The linked survey should take less than 10 minutes to complete. 

As an incentive to participate, you will be entered into a drawing for one of ten $25 Amazon.com gift cards. If you decide to withdraw from the study, you will still be entered into the prize drawing. Anonymous survey responses will be separate from the collection of names drawn for the raffle prizes. 

All responses are strictly confidential and are de-identified – only pooled data will be used. We will protect your confidentiality by turning off all IP address collection processes in the survey software. We will work to make sure that no one sees your survey responses. But, because we are using the Internet, there is a chance that someone could access your online responses without permission. In some cases, this information could be used to identify you. By clicking on the survey link below, you are providing your consent to be part of the study.
