Qualtrics account request process changing

Starting March 3, Ohio State will implement a new process for requesting access to Qualtrics survey software. Any college faculty member, staff member or student needing a new Qualtrics account should visit the OTDI Self-Service portal to complete an access request form. 

Requestors will be asked to verify that they have completed the current fiscal year’s Institutional Data Policy or HIPAA and Institutional Data Compliance training. This training is mandatory before access can be elevated, as Qualtrics has access to S4 data.
